We Welcome Your Referrals

We understand you have a choice for referring to a specialist, especially throughout the Valley.  The quality of primary care in our community is excellent. For those times, however, when you know a patient could benefit by our expertise, equipment or medical intervention, please know that we welcome your calls and will be honored to serve as your health- care partner. We enjoy the camaraderie and collegiality our profession affords by the nature of the referral relationship. If there is anything that we can do personally to assist you, please let us know. We enjoy visiting your practices, providing lunch ‘n learns, or simply sharing a cup of coffee.

Tips For A Successful Referral

Cardiology Referrals

Cardiology Services at DVMS
DVMS offers specialized cardiology care with two board-certified cardiologists and two residents available for appointments. We see patients at our Gilbert and Peoria offices—please call for location availability.
All appointments are scheduled through our Gilbert and Peoria offices, where our team will assist your client in finding the most convenient location.

Medical Records & Imaging Requirements
When scheduling an appointment with our cardiologists or residents, we will contact you to request medical records. We require the past year of medical history, including:

  • Medical notes and physical exams
  • Lab results
  • Previous and current radiographic images
  • Imaging is particularly crucial for cardiology referrals, as it provides valuable insight into cardiac health and progression over time.
  • Diagnostic Testing

Depending on the patient’s condition, Dr. Church may recommend one or more of the following:

  • Echocardiogram
  • EKG (Electrocardiogram) or Holter Monitor
  • Radiographs (if no recent images are available)

If additional testing is required, we will provide your client with a detailed explanation and cost estimate.
For scheduling and inquiries, please contact our Gilbert or Peoria offices.

Our Internal Medicine doctors see patients Monday through Friday in Gilbert and Peoria. For internal medicine appointments, we ask that you send all medical notes, physical exams, lab results, previous and current diagnostic images and associated reports, and any other relevant information. We ask that the patient is fasted, unless diabetic. Water and medications are fine up until the appointment time.

We know that it can be tough for your clients to come to a veterinary appointment during the week. As such, we strive to get all diagnostics and treatment done in the same visit to optimize their time. For internal medicine, it is difficult for us to give your clients an estimate prior to their visit. Multiple factors come into play when we put together their treatment plans. We review the records and test results, take a detailed history, and perform a thorough exam before putting together an individualized, detailed treatment plan. If your client would like a rough estimate before the referral, we ask that you, the doctor, call, and we can discuss the case in detail to assist.

  • Ultrasound & CT Services at DVMS:

    • At DVMS, all ultrasounds and CT scans are performed by our radiologists or sonographers at our Gilbert and Peoria locations.

    • Currently, all ultrasound and CT cases are reviewed through our Internal Medicine department for consultation and further workup.

  • Now Offering Outpatient Ultrasounds in Peoria:

    • We are preparing to launch outpatient ultrasound services at our Peoria office.

    • Outpatient ultrasounds can sometimes cause confusion for clients—once they hear the word “ultrasound,” it often becomes their sole focus.

    • Please be sure to specify whether you are requesting only an outpatient ultrasound or a full referral to our Internal Medicine department to ensure proper scheduling and expectations.

  • Preparation & Medical Records:

    • To ensure the best possible evaluation, we require:

      • One year of medical records.

      • Any relevant imaging previously performed.

    • For ultrasound appointments, we ask that patients do not eat after midnight the night before, except for diabetic patients. Water is permitted until the time of the appointment.

  • Results & Additional Services:

    • Ultrasound reports are provided within 24 hours.

    • We offer same-day teleradiology services for radiograph interpretation by a local board-certified radiologist.
